PLEASE NOTE -- Once you've submitted your Change/Addition of Academic Area form, you must also check with the new departments to which you are applying to make sure you have turned in all of their required materials (departmental applications, test scores, resumes, letters of recommendation, etc.) That supplemental material must be sent to the department.
