Op1.10 Change of Legal Information and Status

Faculty, Staff and Students may change Legal Information maintained by the University by submitting suitable legal documentation to the appropriate offices and indicated in this Policy.

I. Definitions

For purposes of this policy the following terms have the following meaning:

Contact information, means addresses, phone numbers, personal email addresses, and emergency contact information.

Legal information, means name, marital status, social security number, date of birth, sex, guardianship, parent or legal guardian, and/or citizenship status.

Chosen name, means the preferred name designated by a Faculty, Staff, and /or Student through the University's Chosen Name Policy.

Suitable legal documentation, means satisfactory documentation created by an external governmental entity deemed by the University to be sufficient in making a requested change of Legal Information. Documentation sufficient for one request may not be sufficient for another. Suitable legal documentation includes, but is not limited to:

  • driver's license,
  • official state ID card,
  • Social Security card,
  • certified copy of marriage license,
  • court order from a court of competent jurisdiction,
  • current passport or official proof of identity certified by the U.S. embassy abroad or by the appropriate foreigh emabssy in the United States; or
  • any other document required by the State of Missouri to make the requested change

II. Students

The Office of the Registrar manages all student requests to change their legal information. Students requesting a change in their legal information should visit the Change of Student Information page on the Office of the Registrar's website for information on requesting a change.

  • Students are encouraged to keep their Contact Information up to date. Students can update their address through the Office of the Registrar. For other changes to Contact Information, Students can make changes by accessing the “Personal Information” card in My Missouri State. All current and previous names, addresses, phone numbers, and email addresses are maintained as part of the student’s record.
  • Certain records maintained on microfilm cannot be updated. Students who last attended Missouri State University before 1985 likely have records on microfilm and should contact the Office of the Registrar when seeking to make a change in Legal Information. 
  • Changes to a student’s Chosen Name may be completed by accessing the “Personal Information” card in My Missouri State.
  • Students seeking an updated diploma/certificate must first complete the Change of Student Information form. Unless otherwise updated, any reissued diploma/certificate will match the name on the original diploma/certificate unless an official legal name change has been completed pursuant to this Policy.

III.  Faculty and Staff 

Faculty and Staff requesting a change in their Legal Information must contact the Assistant Director -- Compensation and Information Management in Human Resources Office.

  • Changes to an employee’s Chosen Name may be completed by accessing the Personal Information card in My Missouri State.
